Sustainable institutionalization of the Kenyan Health Sector Dynamic Costing Model (DMC)
The current Health Sector programme Kenya is in its first phase running from 01/2010 until 12/2013. It supports the Government of Kenya to implement the national strategic plan in the health sector (NHSSP), the constitutional reform in the sector, as well as the planning in the SWAP that derived from it. Central thematic areas are sexual and reproductive health, health financing, quality assurance and human rights. Within its advisory services on health financing the GIZ Kenya health programme considers the actual and normative costing of health services as a basic prerequisite for any discussion on health systems financing. The “dynamic costing model” is functional and has been used to calculate the fiscal implications of the health financing strategies for Kenya amongst other.
It is still very much a product which is housed, maintained and utilized within the GIZ Health Programme, hence the knowledge and utilization of it outside the programme itself is limited. GIZ aims at supporting sustainable solutions. Therefore the DMC needs to find an institutional home in order to become a self-sustaining product which would be used by different parties. “Housing” it would not mean to serve merely as a repository – in order for the DMC to gain its full utility a whole range of activities outside the housing institution (such as training users, marketing it, publishing on and with it) would need to be conducted. Therefore, the institution would need to absorb over some time the DMC as a product of its own and needs therefore inherent self-interest to use, promote and develop it further.
In order to achieve sustainability the partnering institution will initially need to get some support to have sufficient capacities to carry out some of the functions outlined above. Therefore, GIZ will give some support in the initial period through the consulting company in selected functions.
The consultant is expected to provide an offer which addresses the issue of institutionalizing the DMC and give support to the housing institution. The institutionalization process would address following issues and functions:
In a first step:
— Selection of the adequate housing institution according to predefined criteria.
In a next step.
— Assessing the capacity of the housing institution and proposing capacity developing measures to bring it to the next level (where it can sustain the DMC),
— Strengthening its capacities such as to make it a capable housing institution to carry out the tasks on a self-sustaining basis,
— Defining a training curriculum and carrying it out by training around a hundred participants (amount to be defined in the first period of the consultancy).
